COVID-19 ATTENDANCE FORM – CATERING GUESTS

It is important that you fill in this form correctly in order to help create a corona-free society. Your telephone number or email address is required.
Why are we asking for this?

The National Security Council decided on 23 July 2020 that catering establishments should record the attendance of their guests. There is a good reason: the COVID-19 pandemic is raging once again, and this is just one of the measures taken to better protect you, your family, friends and the other customers. Your health and theirs comes first!

What will happen with your data?

We understand that you might be concerned about the processing of your personal data. We are processing the data on behalf of the AViQ. We will keep your details in a closed envelope for a period of 14 days, after which they will be irrevocably destroyed. Your data will only be further communicated to the authorised government services in the event of a diagnosed infection. Under no circumstances will we use your data for marketing or other purposes other than tracing in the context of the COVID-19 pandemic.

If you have any questions about this, you can ask the person in charge of the restaurant, café or hotel.

From a legal standpoint: on what basis are we processing your data?

You will surely have heard of the “GDPR”: the General Data Protection Regulation. This is a strict European regulation which ensures that your privacy is better protected. We will process your data, directly on behalf of the government, and in line with the decision of the National Security Council, pursuant to Article 6, c (“processing is necessary for compliance with a legal obligation to which the controller is subject”) and Article 3, 10° Ministerial Decree of 24 July 2020 amending the Ministerial Decree of 30 June 2020 on urgent measures to limit the spread of the coronavirus (COVID-19).
